Objective: To describe the longitudinal trend of undernutrition and overnutrition coexistence within the same household among 8 provinces in 1991-2000 and to define their association with the key socio-demographic-economic, dietary and environment factors. Methods: Analysis was based on data of the China Health and Nutrition Survey, an ongoing longitudinal project in 1991, 1993, 1997 and 2000. All households were categorized as four types: undernutrition/overnutrition, undernutrition, overnutrition and normal by the individuals under 60 years old. Multilevel model was used to explore the likely factors in 10 years. Results: The incidence of under- and overnutrition coexistence in 1991, 1993, 1997 and 2000 was 7.9%, 7.7%, 6.0%, 6.8%, respectively. Dual burden households were decreasing (Z=2.54, P=0.0112). Multilevel model analysis showed that urban inhabitancy, owing motor vehicle and washing machine had persistent effect to coexistence of undernutrition/overnutrition individuals within the same household. Conclusion: Undernutrition and overnutrition coexistence within the same household is one of the characteristics of Chinese nutrition transition. The nutrition improvement program in the same household should be paid attention to both situations.
